The first season of the American medical drama television series The Pitt premiered on January 9, 2025 and concluded on April 10, 2025. It consisted of 15 episodes, each running from 40–53 minutes in length. Max broadcast the first season on Thursdays at 9:00 pm in the United States.

On the fourth anniversary of the death of his beloved mentor Dr. Adamson during the COVID-19 pandemic, Dr. Michael "Robby" Robinavitch starts a shift at the Pittsburgh Trauma Medical Hospital ER (nicknamed "The Pitt"), relieving overnight attending Dr. Abbott. Robby meets his new students and interns: Victoria Javadi, daughter of surgeon Eileen Shamsi, who faints when confronted with a degloved foot; insecure but caring Dennis Whitaker, who injures his finger during a patient transfer; and the confident and assured Trinity Santos. Also new to The Pitt is Mel King, a socially awkward but skilled resident who is assigned to shadow charismatic and self-confident senior resident Frank Langdon. Hospital administrator Gloria warns Robby that the Pitt must improve its patient satisfaction scores. Charge nurse Dana Evans discovers that senior resident Heather Collins is pregnant, but agrees to keep it a secret. Collins tries to communicate with Minu, a patient who was pushed onto a subway track and does not speak English. Cassie McKay, a second-year resident, treats a triathlete who presents with shortness of breath. Robby meets Theresa, a mother who made herself ill in order to seek help for her son David after discovering a list of girls David wants to hurt. David flees before Robby can discern his intentions.

When a four-year-old child is brought into The Pitt after accidentally ingesting one of his father's weed gummies, Langdon and King have to manage his irate mother while ensuring the family receives the required intervention from Children's Services. Robby must go against his morals when the children of an elderly man with pneumonia, Mr. Spencer, decide to override his do-not-resuscitate orders and pursue treatment that could lead to a slower, more painful death. Other cases include an unhoused man who arrives with a coat full of rats, a man with an electric dog collar glued to his neck, and a man who got into a scooter accident while not wearing a helmet. Javadi clashes with Santos, and learns about McKay's life as a single mother and doctor, but is shocked to find that McKay wears an ankle monitor. Third-year resident Samira Mohan correctly identifies that a woman who presented with crippling pain was not a drug addict seeking a fix but instead dealing with sickle cell disease. Robby struggles with telling the parents of Nick, a teenage boy, that their son is brain-dead after he overdosed on Xanax laced with fentanyl. Whitaker connects with Mr. Milton, a patient whom he later finds unresponsive in the hallway.

Despite Whitaker's best efforts, he is unable to resuscitate Mr. Milton, and struggles to process his first-ever death of a patient. Javadi unsuccessfully tries to learn more about McKay from Langdon. Robby orders unnecessary tests for Nick to give his parents time to accept the inevitable. Nick's father clashes with one of Javadi's patients, Jenna, who took fentanyl with Nick but has recovered. Minu works out that Collins is pregnant, but she remains guarded when discussing her personal life with Robby. Collins trusts Javadi to complete an intubation on a man who has been shot in the chest with a nail gun and she succeeds, much to the chagrin of Santos, who frustrates the team with her brash and overconfident personality. Robby clashes with Mohan over her patient-focused but slow work pace, warning her not to be so afraid of making a mistake that other patients go untreated, advice she tries to pass on to Whitaker. Hospital social worker Kiara encourages Robby to get David back to the ER by any means necessary; McKay agrees, having had negative experiences with violent men in the past. Mr. Spencer rips out his breathing tube and has to be restrained while his daughter continues to push for further treatment in the hope her father will improve. Eventually, his children agree to withdraw treatment and allow him a less traumatic death.

The nurses place bets on the whereabouts of a stolen ambulance, and follow its progress on the news. Robby continues to monitor Mr. Spencer in his final hours and is unable to push the thoughts of Adamson's death from his mind. He imparts advice he received from Adamson to Mr. Spencer's children, who grow closer during the heartrending wait for their father to pass. King and Mohan treat a five-month-old baby, and support a single father when his daughter is discovered to have an imperforate hymen, while Collins prepares to administer a medical abortion to seventeen-year-old Kristi. Jenna tries to make amends with Nick's family. Santos's poor bedside manner with Jenna leads her to criticism from Mohan, and she also experiences a setback when she orders BPAP for a patient without clearing it with Langdon, which almost results in the patient's death. Dana and Whitaker have a challenging experience trying to medicate a distressed mental-health patient, while Javadi impresses McKay when treating a trans woman with a cut on her arm. Mateo, one of the nurses, experiences racist abuse from Doug, a patient who has been stuck in the waiting room for hours.

Mohan earns Collins's praise for her empathetic practice with her sickle-cell patient. McKay reveals that she is a former addict who had previously lost custody of her son, which encourages her patient to open up about her own struggles; however, Javadi's naive overstepping causes the patient to leave, and McKay warns her of the importance of listening to patients. Jake, the son of Robby's ex-girlfriend with whom he has a familial relationship[3] briefly visits The Pitt and is welcomed by the other staff. When Langdon performs an unconventional airway clearance on a young man whose recent tonsil surgery becomes a life-threatening hemorrhage, Robby informs him that he has recommended him for a prestigious upcoming fellowship. Whitaker receives positive feedback from King, Robby, and Mohan about his work with several patients. After a disagreement with Langdon over how best to stabilize a man struck by a seizure, Santos's confrontational personality is called out again, and she considers complaining to Robby. King treats Ginger, an older lady brought in following a fall, and offers advice to her daughter who shows signs of caregiver fatigue. Ginger's daughter goes to park her car and does not return. Collins discovers that Kristi is one week over the legal cutoff for a medication abortion, but Robby takes over and manipulates the ultrasounds so that the fetus appears just under the cutoff age. However, Kristi's mother Eloise barges in and forbids the abortion from taking place.

The stolen ambulance crashes, and the team treat the two frat boys responsible. Santos is humbled when she accidentally drops a scalpel into the foot of Yolanda Garcia, a surgeon to whom she had been ingratiating herself. Nick's parents continue to struggle to accept the death of their son, while Collins treats a young man with testicular torsion and is encouraged by his relationship with his mother, which helps her start to come to terms with her own pregnancy. Santos and Langdon treat a patient with neurocysticercosis, as King struggles with what to do about Ginger, who is still waiting for her daughter to return. Javadi is embarrassed when her mother arrives to see a patient. Gloria tells Robby that, if patient satisfaction scores don't improve, she will have to consider bringing in a corporate management firm to take over The Pitt. Kristi locks herself in a bathroom and refuses to leave with Eloise; Eloise and Kristi's aunt Lynette fight outside the bathroom and shove Collins away when she tries to intervene.

Following a frank discussion with Collins, Kristi's mother consents to the abortion. King's experience caring for her sister is an asset when Langdon has difficulty supporting an autistic patient with a sprained ankle. The team treat a gentleman suffering from a heart attack and call in the ECMO team for assistance, which triggers Robby's memories of Dr. Adamson receiving the same intervention. He later argues with Collins over his treatment of Mohan and the other staff. Mohan and Javadi treat an influencer with suspected schizophrenia, but Mohan disregards instructions, carries out further investigations and discovers that the patient has mercury poisoning from a contaminated face cream. The wife of Santos's patient confesses that she has been drugging her husband with progesterone to dull his sex drive because she believes he is molesting their daughter. When the daughter does not want to discuss the situation, Santos threatens the incapacitated father, citing previous experiences with men who abuse their positions of trust. Collins goes to the bathroom and suffers a miscarriage.

Collins returns to work without disclosing her miscarriage. The team are unable to save Amber, a six-year-old drowning victim. McKay and Dana grow concerned about the relationship between Piper, a young woman with chlamydia, and her overly-controlling boss, believing she may be a victim of sex trafficking. Santos shares concerns about Langdon's handling of Librium with Garcia, who dismisses her concerns. Javadi impresses her mother by determining that one of her patients' symptoms were caused by a spider bite rather than her Crohn's disease. A patient with Twiddler's syndrome reveals he was part of the Freedom House Ambulance Service and was a friend of Dr. Adamson. When informed of Adamson's death, he mourns but stays positive in emphasizing how much he contributed to the world. Ginger's daughter returns, having fallen asleep in her car, and King and Kiara help them to access homecare services to ease her workload. Nick's family agree to organ donation, and Robby and the rest of the staff hold an honor walk as he and his parents leave the hospital.

Collins confides to Dana that she suffered a miscarriage, but returns to work anyway. Robby tries to console his team after the death of Amber. McKay makes a final attempt to have Piper admit to being sexually trafficked, but fails, and Piper is discharged alongside her "boss". Dana has to intervene as a fight breaks out in the waiting room, where a woman loses a tooth and both women must be admitted to the ER. Santos makes a quick call to stop an MDMA overdose patient from seizing, but her decision to act alone and not call in Langdon enrages him, causing him to launch into an aggressive and personal verbal attack on Santos. In turn, Robby lectures Langdon on his behavior, and failing to act as a better teacher to Santos. Whitaker receives a heartfelt apology from the homeless psychiatric patient who urinated on him, leading to him decide to join The Pitt's "street team". Two victims of a car crash are admitted to The Pitt, including Paula, who was seen by McKay earlier in the day. Paula is discovered to have blacked out while driving due to sepsis caused by endometritis, and Collins suggests that McKay may have missed the septicemia in Paula's earlier exam due to fatphobia. Doug, the patient who complained of long wait times, threatens reception staff and leaves without medical clearance. As Dana goes outside to smoke, Doug sucker-punches her to the ground.

Robby treats Dana for minor injuries. The nursing team is left shaken by the incident, and Gloria is unable to allay their concerns. Robby chastises McKay when he finds out she called the cops on David, but she remains adamant that she did the right thing. A whole-body burn victim arrives in critical condition, and Whitaker tries to support the victim's emotionally devastated wife, despite a pessimistic outlook from Langdon. McKay's son Harrison arrives at the ER alongside his father, her ex-partner Chad, who has been injured in a skateboarding accident; Chad's new girlfriend Chloe is part of the reason for McKay's ankle monitor. King and Mohan bond as they treat a young stroke victim with TNK, but King becomes concerned she lacks the "special sauce" to become a great doctor. Javadi becomes angry with the father of a teenager who was hit in the eye by a baseball, feeling he is prioritizing his son's baseball career over his health concerns. Tensions continue between Santos and Langdon, with Robby struggling to mediate. After Santos tells Robby her concerns about the Librium, Robby investigates Langdon's locker and finds several stolen doses. Livid, Robby sends Langdon home.

Collins and King help a surrogate mother with delivery. Collins opens up to Robby about her miscarriage and an abortion she had while they were dating. He comforts her and Collins takes Robby's advice to go home early to relax. A disagreement between Mohan and Whitaker arises when they treat a man who Mohan believes is seeking morphine. Santos is concerned with the repercussions of reporting Langdon and speaks to Garcia and Robby about her concerns. Robby repeatedly refuses to answer phone calls from Langdon. McKay and Javadi treat a woman who experienced an esophageal hemorrhaging while in recovery. She is treated by draining the blood and using a medical balloon to stop the bleeding. Robby supports Theresa to petition for David to be placed under a psychiatric hold. McKay confronts Chloe over custody of her son. Dana tells Robby that she is leaving the Pitt after being punched. News comes in from Pittfest, a music festival that Jake and his girlfriend Leah are attending, that there is an active shooter there.

The hospital activates its mass casualty incident protocols to deal with the influx of patients from the Pittfest shooting. Abbott and other staff members start their shifts early, while Shamsi, Garcia, and other doctors from other departments assist the ER. Langdon returns to help, to Robby's consternation. King discovers liver damage in a patient with a broken leg, whose deaf son is in surgery for gunshot wounds; she donates her own blood to keep the patient alive until blood bank supplies arrive. Faced with a shortage of chest tubes, Javadi improvises a substitute, surprising her mother. Police inform the team that the shooter could be heading their way; Robby and Theresa fear it might be David. Santos discovers one patient is actually a photographer faking injury to gain access to the Pitt; fleeing, he slips on the bloody floor and hits his head. Chad comes looking for Harrison, and is stunned by the carnage and chaos; McKay sends him to wait in the lounge with their son. Robby is unable to contact Jake. After an hour, the rush of incoming patients shows no signs of abating.

New blood supplies arrive by helicopter as the number of patients exceeds 80. Hospital staff continue to use unorthodox and unfamiliar procedures and equipment to treat patients. Intraosseous infusion drills see many uses: Whitaker unwisely (and painfully) uses one to administer medication to a conscious patient; Mohan uses one to relieve another patient's high intracranial pressure; and when McKay's ankle monitor alarm sounds, she uses a drill to disable the device. Kiara and ward clerk Lupe photograph the dead in order to identify them and notify their families. With no senior physicians available, Santos takes the initiative to perform a REBOA to stop a patient's bleeding; Abbott scolds her for her rashness but praises her for the procedure's success. David returns to the Pitt and is immediately tackled and handcuffed by police, but protests his innocence. Jake arrives with Leah in critical condition; Robby attempts heroic measures to resuscitate Leah, using up several liters of blood over the protests of other staff members, but is unable to save her. After granting Jake's request to view Leah's body, Robby is overwhelmed by grief and guilt over Leah, Adamson, and other patients he lost, and suffers a panic attack.

Whitaker helps Robby get through his panic attack; Robby returns to work but is noticeably short-tempered with his patients and colleagues. One of the last PittFest shooting victims to arrive is a man who stayed behind to help other victims, and developed an air embolism in his heart; with Abbott's guidance, and over the objections of a surgeon, Mohan performs a risky procedure to save him. Santos and Langdon are thrown together while treating an overdose patient. McKay's father arrives to pick up Harrison. As the PittFest crisis slows down, the Pitt begins transitioning back to normal operations. The shooter is reported dead and David is uncuffed, but still on 72-hour psychiatric hold, which leads him to clash with his mother, Robby and McKay. An unresponsive minor, Flynn, and his sister are brought to the Pitt, and Robby diagnoses him with the measles and probably acute disseminated encephalomyelitis (ADEM); when Flynn's mother, relying on half-understood health information on the Internet, refuses to allow a lumbar puncture to test for ADEM, Robby loses his temper with her. The police arrest McKay for destroying her ankle monitor.
